Office Closure
Effective 
January 31, 2026

Dear Patients,

After much consideration and with deep gratitude for the opportunity to serve our community, we have made the difficult decision to close our Moscati Center office location at 2040 Highway A1A, effective January 30, 2026. Services at this location will remain available through that date.

โ€‹

While this office is closing, all services will continue to be available at our Kissimmee, Florida location at JMJ Family Practice. We encourage patients to consider their care options. If you choose not to continue care at our Kissimmee location, please begin the process of transferring your care to a new provider as soon as possible.

โ€‹

Requests for medical record transfers must be signed in person or notarized and submitted prior to January 30, 2026.

โ€‹

Please note that all appointments scheduled for dates after January 30, 2026, have been canceled, effective immediately.

โ€‹

Medical Records Requests

โ€‹

Please note that we do not release medical records without a signed consent from the patient or the patient’s parent/legal guardian. Additionally, medical records are not released via email.

โ€‹

Medical records may be requested in the following ways:

โ€‹

1. In Person at Our Office
A medical records release can be signed in the office. You may request:

  • Printed copies of your records, or

  • Records to be faxed to a new provider

Please be sure to provide the name and fax number of the receiving office. A valid photo ID is required at the time of signature.

โ€‹

2. Request from Another Provider’s Office
You may sign a medical records release with another provider. That office can fax the signed release to 407-935-9108 for processing.

โ€‹

3. Notarized Medical Records Release Form
If you are unable to come into the office, you may sign the medical release form in the presence of a notary. The notarized form may then be:

  • Faxed from a secure fax line to 407-935-9108, or

  • Mailed to our office

Please ensure the notarized form is received prior to January 30, 2026.

If we do not receive a medical records request prior to January 30, 2026, records may be requested from our Kissimmee office with a signed medical release, as outlined above.

Medication Refill Requests During Transition of Care

While considering your future care, if you decide not to continue care with our Kissimmee, FL location, we understand that you may need medication refills while you transition to a new provider.

โ€‹

Medication Refill Options:

  • If you have been seen in our office within the past 6 months:
    Please let us know which medication refills you need, and we will process refills for up to 90 days for most medications. Prescription request must be only for medications we have prescribed for you in the past. 

  • If you have NOT been seen in our office within the past 6 months:
    You will need to be seen on or before January 30 for medication refills to be processed during your transition of care.

Please note: These options do not apply to controlled medications categorized as schedule 2 3 or 4. For information regarding controlled medications, please contact our office directly.

Notice Regarding Patient Portal Access

โ€‹

Effective January 31, 2026, patient portal access will be discontinued for patients assigned to the Moscati Center as their primary office location.

โ€‹

Patient portal access will remain available to only those patients who have elected to continue care with our Kissimmee JMJ Family Practice location.

โ€‹

If you have questions regarding your primary location or require assistance during this transition, please contact our office for further support.

Coming Soon: Telehealth Appointment Options

โ€‹

Dr. Fernandez and our providers are working toward offering telehealth services to continue care for patients who are unable to travel to our Kissimmee, FL location.

โ€‹

This will be a cash-only service and cannot be billed to or reimbursed by insurance. Patients who choose this option will be required to sign waivers acknowledging the payment policy, telehealth guidelines, and non-reimbursement clause.

โ€‹

Telehealth appointments are expected to begin booking in mid-January for February appointments.
